Since we know the slope, all we need to do is find the y-intercept of the line.

y = mx + b

6 = 1/4(4) + b

6 = 1 + b

6 - 1 = 1 - 1 + b

5 = b

Now, put into the final equation.

y = 1/4x + 5

Best of Luck!
